Wealth Advisor Benefit Plan

• $320 per participant

• $200 annually in November of each year
• $12/month thereafter through the end of the plan year
• $350 annually for the Advisors Forum membership

If you wish to cancel/change coverage, it must be due to a qualifying event and you must provide a 30 day written notice. Failure to do so will result in premium taken for that month. Any drafts returned, will results in an additional $40 fee.
